• Required Medical forms for Summer Camp
  • Emerald Bay Medical Information Sheet (pdf file)  Describes medical forms required for Emerald Bay.

  • Class 1 Medical Release Form (Acrobat Doc)  Required for Scouts for summer camp.  Class 1 part to be filled out annually by parent. 

  • Class 2 Medical Release Form (Acrobat Doc)  Required for Scouts for summer camp.  Class 2 part to be filled out every 36 months and signed by a  licensed health-care practitioner.

  • WLACC Youth Permission Form (Acrobat Doc) Required for Scouts for summer camp.  Includes camp permission to treat, firearms release, photo release etc.

  • Class 3 Medical Release Form (Acrobat Doc) Required for leaders for summer camp and all for high adventure outings.  This form is usually filled out just prior to such an event.  Requires a medical examination and signature of licensed health-care practitioner.
