Annual Medical Forms Update

Medical Release Form (aka Permission to Treat Form)

For all Troop 22 outings a current Permission to Treat form is required for each Scout.
Without this form, we can not get medical attention for an injured Scout beyond basic first aid.
It is critical that this information be current and up to date!!
Online versions of this form:
- Medical Release Form (Acrobat Doc) To print and fill in by hand. You need the free Acrobat reader on your computer to read this file. If you don't have it you can get it by clicking on the "Get Acrobat Reader" button on the left panel of this website.
- Medical Release Form (Word Document) Here is a Microsoft Word version of the Medical Release Form so you can type in your information and print out and sign each year.

Because many companies change their insurance benefits each year in January, we have chosen January as the month for the complete troop update of these forms.
This reminds those whose medical insurance has changed to let the Troop know (not something most of us think about).
For those whose medical insurance has not changed, we simply need to secure a current signature on the form.
